Work Permit Application

• Work Permits are only processed after school closes for the day at the Stoughton High School Office.  

• ALL STUDENTS aged 14 -17 must first obtain a completed Promise of Employment section of the Work Permit form, which is to be completed by the Employer, a Physician, and the Parent/Guardian (students aged 14 & 15 only).

• ALL STUDENTS seeking a work permit must have a birth certificate or an official document showing their date of birth.

• ALL STUDENTS must be present when the permit is issued.
